CHS Welcomes Latest Providers to Join its Medicare ACOs for 2023

As CMS Medicare ACO deadlines approach, CHS helps more providers achieve savings and improve care

TAMPA, Fla., June 13, 2022 /PRNewswire/ — Collaborative Health Systems (CHS), a management services organization, is welcoming recent partners to take part in its value-based care models in 2023. Although some application deadlines have passed, Medicare providers still have until August 4, 2022, to have interaction CHS and join its Accountable Care Organizations (ACOs) within the Medicare Shared Savings Program (MSSP) and the ACO Realizing Equity, Access, and Community Health (REACH) models.

For greater than a decade, CHS has helped healthcare providers navigate and reach value-based models including MSSP, Next Generation ACO, and Global and Skilled Direct Contracting (GPDC). Last 12 months, CHS expanded its footprint to 10 recent markets and is anticipating continued growth this 12 months, as interest in ACOs stays high. Through participation in those models, Medicare providers are encouraged to coordinate care to enhance outcomes and reduce spending, which opens opportunities for increased revenue. Because the Centers for Medicare & Medicaid Services (CMS) continues to evolve its value-based care models similar to the recent transition of GPDC to ACO REACH, CHS helps its partner practices navigate this ever-changing landscape.

“The transition to value-based care alters care management at a foundational level. It is vital to begin conversations as soon as possible, so we are able to guide providers through the crucial first steps to maximise impact,” said Anthony Valdés, President of Collaborative Health Systems. “Our collaborative process engages providers every step of the option to simplify practice management, enabling us to drive results above industry averages 12 months after 12 months.”

Medicare providers who partner with CHS face no upfront or monthly program fees as they transition to value-based cared models. A dedicated local CHS team works with partner providers to grasp regional population health challenges and collaborates with care processes and workflow. Consequently of partnering with CHS, partner providers can deal with their patients and deliver higher health outcomes. Since 2012, CHS has helped partner providers reduce unnecessary hospitalizations and ER visits, while achieving greater than $475 million in total savings to the Medicare program and achieving a median overall quality rating of 97% in 2020.

“We consider Medicare beneficiaries can enjoy higher health after they have a robust relationship with a primary care provider who has the time, information, and resources to supply high-quality care,” said Michael Barrett, Vice President of Strategy and Development for Collaborative Health Systems. “Our strong leads to supporting successful value-based contracting are a testament to the impact high-quality care holds for all.”
